Hospital Staff, Social Workers and Nonprofit Partners

Hospital staff, social workers and nonprofit partners can connect heart families directly with Linked by Love. A warm referral removes another task from a parent’s already-full plate and creates a clear, trusted path to ongoing support.

Please confirm that the family has agreed to be contacted before submitting a referral.

  • 1. Refer a heart family.
    With the family’s permission, select the button below to begin the referral.

    2. Complete the short referral form.
    Share the family’s basic contact information and where they are in their heart journey. Detailed medical information is not required.

    3. We’ll reach out personally.
    Linked by Love will email the family a warm welcome and a private link to create their username and password.

    4. The family receives immediate access.
    Partner-referred families do not need to complete an additional application. Once they create their account, they can access our private support groups, wellness programs, heart-family events and trusted resources.

  • Families are often handed long lists of resources while managing appointments, hospital stays, surgeries and everyday life. A direct referral turns information into a meaningful connection. Instead of asking the family to search for support or complete another application, we bring the next step directly to them.
